The information below are provided as a service to our patients

APPOINTMENT POLICY (We see both referred patients and those who are self referred)

When we schedule an appointment for you, we will do our best to see you promptly. if an emergency occurs with another patient, you may be asked to wait beyond your appointment time. If this is inconvenient, please let us know and we will reschedule your appointment.

If you need to change your appointment or leave information when the office is closed, please call the office and leave a message on the answering machine. If, in fact, you find that you are unable to keep your scheduled appointment, please give our office at least a 24 hour advance notice.

Our phones are answered by our office staff from 8:00 a.m. to 5:00 p.m. Phone messages taken by our personnel will be returned as promptly as possible. The physicians are not always available in the office as they are performing procedures or seeing patients in the hospitals. We thank you for your patience and understanding.

EMERGENCIES
In the event of a life threatening emergency after office hours, please call 911 or go to your local emergency room. When you arrive at the hospital, please ask the emergency room physician to contact the physician on call for our practice.

In the event of an emergency that you need to contact the physician on call for our practice, please call 208 234 2500.

PRESCRIPTIONS

If you need your prescription refilled, please contact your pharmacy and instruct them to call our office : (208) 232 6616

They will have all of the pertinent information needed to properly refill your medications. Please call for refills before 3:00 p.m. Monday Friday if at all possible.

PAYMENTS

As a courtesy to our patients, we will file your insurance claims to help you receive the maximum reimbursement under your policy.

Where there is an office co-payment due you will be expected to pay at the time of visit. Also, where insurance plans permit, you may also be asked to pay co insurance amounts at time of visit.

You will be billed for any remaining balance after claims are processed and contract adjustments have been made. Statements are mailed monthly to patients with an outstanding account balance.
